Travel Information Form - Instructions

1. Download Adobe Acrobat Reader if you don't already have it on your computer
2. Click here for the Travel Information Form
3. Fill it out and click 'submit' for sending by email or fill it out and print, then mail to us at the address above.
4. Don't forget to indicate 'am' or 'pm' for your flight arrival and departure times
5. For travel and rendezvous arrangements other than flights, please put your details in the 'Special Needs or Travel Plans' box
6. If you're carrying a cell phone while traveling and would like us to be able to contact you, be sure to give us your number.
7. IMPORTANT!  We must have this info at least 45 days before your tour departs.
